I would bid 1♠ 2nd seat with such a fine vulnerability. I saw many experts adjusted Rule of 20 to Rule of 19. (5 ♠ + 4 ♦ =9 +10 HCP = 19)
In Pau, France the 49th edition of the European Team Championships of all categories players worked out for same deals same day.
Tho in general all went well; as a matter of style similar 21-19 positions sometimes passed out. Above post taken 1 or maybe 2 weeks ago (If I remember correct) which dealt on BBO.

What I'd do there depends on how I'm feeling psychologically that day but I don't know what's right or wrong, I've never thought extensively on 4th seat openers.
Thread hijack? Is this a clear passout at MPs?
From here I don't see much point at opening it in IMPs anyways.

I think this depends a lot on your opening style in 1st/2nd seats. If you tend to use a light style opening, then this is a passout, regardless of form of scoring.
If you happen to use a sound opening 2/1 style in 1st/2nd seats, then this hand is an absolute must to open in 4th seat to protect partscores and even games for your side, at both forms of scoring.

I'd open East because I have a great rebid and because the AK109 combo bumps up one point, IMO.
I'd open South because I overbid with spades a bit.
I'd open West 1♥ because I have playing strength and good reason to want a heart lead.
I'd pass North because I would open the other three hands and therefore can expect South to be slightly weaker than most would expect.
Not playing this way, I might pass all three first seats but then open in fourth seat.
I think it is either 3:1 or 1:3 if not 0:4.
