1) Supposing it goes pass-pass-pass, what do you lead?
2) Supposing it goes pass-pass-double-pass back to you,
A) What kind of hand to you expect from partner, and
B) If you leave 4HX, what do you lead now?

I'd lead club in both auctions.

I assume 4♥ is a (strong) single suited hand. I assume partner will NOT double in balancing seat with ♥ length <-- basic discipline. 4th seat's double of 4♥ in direct seat would be penalty.
Thinking about what partner might have:
1) Good hand, but wrong shape for action over 3♣: 4441 w/ stiff not in ♣, 2443 with one 4-card major and 3 ♣ or a 4333 hand not worthy of 1NT overcall. With 5+-card suit presumably partner will overcall.
2) Fair hand, too weak for an initial takeout double over 3♣. 4432 or 4441 w/ ♣ shortness.
3) Fair to light hand with other 2 suits. We can discount 3)w/ 55 or 45 patterns since partner would likely overcall 4♠ in balancing seat. If 4♠=6♦ partner would (should) start with a 3♦ overcall. However 4=3=4=2 is possible (see 1).
Not knowing your partner's affection for discipline, I would trust partner has 4=3=4=2 and opening values. High level doubles like this should not have trump shortness, rather should be more cooperative shape.
To the OP:
1) With no double I lead 3rhd highest ♣. Perhaps partner is short or better, we exhaust a dummy entry before it can be put to good use.
2) With the double, and the 4=3=4=2 shape I suspect from partner, I lead {spades]A then read partner's signal.
Since I do not think our side has 11 tricks but we do have 4 QTs, I pass partner's double. I hope to score ♠AK and a ruff, and 2 minor Aces at the very least. Partner should contribute one if not 2 tricks more.

Partner turned out to have a weakish 5-3-5-0 and hoped his double was takeout.
Unfortunately, I was fingering a club before he doubled, but after the double, I decided on ace and another spade... dummy had Kxx xx x AQTxxxx. Declarer won trick 2, pulled trump, played 4 rounds of clubs, pitching his last diamond as I won the CJ. Minus 690. Yum.
I am encouraged the whole world didn't say "bid 5D" or "obviously lead the club."

I'd expect partner to have a weakish 5-5 in the unbid suits, so I'd bid 5♦.
... that would still not be conclusive proof, before someone wants to explain that to me as well as if I was a 5 year-old. - gwnn

The bidding needs a lot of work obviously but so does the defence.
That dummy is too strong for pard to own their bid. After A♠ (deuce from pard) low♣, ♦ back, another ♣ and Bob's your uncle.
I would lead a club on the go without the double.
When a deaf person goes to court is it still called a hearing?

I would lead the ace of spades, but there is no way that I continue this suit.
