Trumps are spades, south needs 5 tricks.

South draws the last trump with the ace and west must throw a heart, which allows south to lead the ♥J and use only one entry to establish the hearts.

The position can be simplified by removing a diamond for each player. edit: no it can't, otherwise West hops up with ♥K and returns ♦Q

With only West squeezed, is this not just a three-suited simple squeeze (possibly without the count??)? The ruff to take the eventual heart finesse is neat though.

West can throw a diamond on this position, since east guards it, otherwise the position is as in the OP. Now cash the ♦A and ruff a club before finessing the heart. West has to win and give you the remaining tricks, as he has only hearts left.

You can't be sure of making if you start by cashing ♦A

The diagram on the left illustrates a basic shape-shifter squeeze-strip at notrump.

You (South) cash ♠A squeezing LHO. A club discard by LHO is immediately fatal so he postpones his fate by discarding a ♥. Now you can safely cross to dummy with ♥A to finesse ♣s.
