4th suit at the 4 level is typically best used as a good raise. If Responder had wanted to introduce diamonds they could have done it the last time around. It is not like a quantitative 4NT rules out finding a diamond fit either assuming natural continuations rather than any sort of ace-showing (ie 5♦ now is usually going to be a max 6034).
