I don't think you need to wonder about what to do when someone bids as appallingly as West. It should not happen very often.

Then East can put in the not unreasonable 3♣, then assuming opponents still bid to 5♦, the heart lead is really obvious.

Actually I was the one who bid 5♦ - I took my partner for extreme heart shortage after he pulled the double and so I hoped for the ♥A lead ruffed in dummy.

Freak hands are tough and it's a realy fine line between raising 4♦ to 5 (your ♥ ruff in dummy could be their overuff) but it's not a juicy penalty missed at all.
East will (or should) run to 4♣ or even 3♠ in passout seat and you ain't getting rich or even plus if you misstep.
That said, passing out 3♥ is equally tough.

the first question is, what is the meaning of the X?
The X occurred in the passout, but after both sides did limit
their hands.
I would say, tha X is pure penalty, so X is fine, it is even
matadory.
Responder has no reason to run, he has no bad surprise for
opener, in fact the Kx in partners suit is ok - it is a ruffing
value.
