Simple question really. Also does the answer change if it's a minor + spades instead of hearts + spades?

when responder bids 3h (even if they originally planned on making
it an invitational bid) the discovery of the double fit moves the partnership
from invitational to game forcing. When opener's original suit is a minor
the partnership requires a significant increase in playing strength to play
5 of a minor. I would use the 3m raise to show invitational values and an
alternative safe place to play vs a possible 43 M fit. If opener has 4 card M
support I would then consider the pair of hands game forcing.

I can't speak for standard, but I can speak for what makes sense to us.

1H-1S
2S-3H...Responder has four spades and invitational values with 3H. Opener might have been pressed for a rebid and chosen to raise spades with only 3. Responder's 3H is an attempt to sort out whether we belong in 3H, 3S, 4H, or 4S.

I think standard is that repeating old suits is NF. So this auction is NF, but 3m from responder would've been forcing. I prefer to play this auction forcing though; would raise hearts directly with an invite.

In standard if you had an invitational hand with a heart fit, bid 3. If you bid 1 first it can only be because spades might be a better place or that you are weaker/stronger than inv. If partner supports spades and you want to go back to hearts, it can't be spades was the much better place. And if you want to bid again it can't be that would were weaker than inv. Therefore this should show a heart fit, a spade suit, and more than invitational.

Do you mean 1-2m; 3m-3 or 1m-1; 2-3m? I play both of these as forcing.

The hand that spawned this was A983 983 AQ72 75. I had suggested that I would respond 1S to 1H knowing that 1H - 1S - 2S - 3H would be a slight overbid, and my companion said it wasn't an overbid because 3H wasn't forcing. Several players bypassed the spade suit entirely to just show a 3-card limit raise via forcing 1NT. (In practice there wasn't a double fit so you always ended up in 3 or 4 hearts depending on whether partner accepted.)
