It asks for the queen of trumps - here, spades. Several different response schemes exist. 5 denies the queen, higher bids show it. But which exact higher bid to use depends on agreements. A common agreement is to immediately also answer with the queen as if partner asked for kings.

P.S. I wrote this under the assumption that 4NT was RKC Blackwood for spades, but this seems to be false. Sorry.

It usually asks for the Q of trumps to avoid playing a slam in a supposed 8-cd for missing a key and that Q.

However, playing R&KCB, your answer to the BW should have been 5S, 2 keys and the trump Q. The 5 keys being the 4 aces and the trump K.

Maybe when you play regular 4 aces BW it also asks for the trump K?

Most likely it means this was a pickup partnership where North was playing RKCB and South was not.

I imagine that OP assumed that 4NT was plain Blackwood asking for Aces and is not familiar with RKCB follow ups.
